Registration is limited to 50 participants due to social distancing requirements. Registration link provided below.

Workshop (90 minutes workshop including Q&A)

Create your own Health Care Proxy and Living Will – Educational Workshop!
by Girish A. Mehta, Founder and Executive Director of Indian Circle for Caring USA Inc., (ICC)

On Sunday, January 30th, 2022 at 2 PM EST
At Evergreen Adult Day Care Center, Woburn, MA
15 Commonwealth Ave Suite 110, Woburn, MA 01801

During this workshop, you will learn about “Health Care Proxy and Living Will”, also known as “Advanced Health Directive” and prepare this important legal document (non-financial) at no cost to you and without further delay. It will be a “Do it Yourself” (D.I.Y.), confidential and without any legal fees project.

During the workshop, you will

  • Understand the need for and considerations in preparing this important legal document required for all adults; men or women; in good health or poor health.
  • Develop understanding and express your wishes about YOUR health care preferences.
  • Receive a FREE copy of Five Wishes document for each attendee prior to the webinar.
  • Save hundreds of dollars ($$) of legal fees and avoid delays.
Registration is required and you will receive a copy of personal self-assessment as well as a link to download a copy of the FIVE WISHES document for you to use during the workshop. Registration is limited to 50 people due to social distancing requirements, so register early to reserve your spot.
Five Wishes is changing the way we talk about advance care planning. It’s more than just a document. Five Wishes is a complete approach to discussing and documenting your care and comfort choices. It’s about connecting families, communicating with healthcare providers, and showing your community what it means to care for one another.

Everyone above the age of 18 should have one created for themselves. Have a peace of mind and do not be caught unprepared. Please take advantage of the unique opportunity, register and attend. This is a free workshop as a service to the community.
